To me, this makes it pretty clear that something is wrong with your proximity sensor or the connection to it. I don't have any idea if those are hard to come by these days, but I think it sounds like the culprit.

Obviously if could be something wrong with the connection on the motherboard, but hopefully that is not the case.

In past month, I replaced battery, and have same issue. Your proximity cable was broken. I oredered one proximity cable & replace and device working as new.
