I don't think the cliche flies anymore...I have seen (not as many ...sure...but their numbers are growing) other Nationals who fit the clinically defined medically grossly-obese bill very well...fellow Canadians, Mexicans, Chinese, South Koreans, Brits, South Africans, and many more.
