No, the tap-and-hold menu hasn't been removed, and it isn't controlled by a setting. It should always be enabled.

Unfortunately, it would be impossible for Maemo Mapper to automatically download those maps, without code changes. The tile numbering is too King-County-specific.

Satellite maps might be a lost cause from Google. Theyir spyware-detection algorithms are too sensitive to consider Maemo Mapper requests to be legitimate. I can't even test it in my browser...
